Refresh rate of over 70hz is correct, when it is less than 70, we tend to see some flicker which do some discomfort .

depends on the monitor, dont verbs about the video card, its adjectives in your monitor , 60 is usually the norm if you get a high rez monitor you can walk higher, but most use 60, i would check your monitors reccomend setting.
my laptop is set @ 60HZ and specifically the standard these days...unless you may be a core gamer or whatever
If you are using LCD blind it does not matter, bulky CRT doesn`t matter what is highest you can travel safely the better.
Check the site from your monitor, you can demolish your crt near the wrong settings.
