Hmmmm... Three things.
a. Broken Sound card.
b. Botched installation of the DirectX suite.
c. Botched installation of sound card drivers.
I'd beggin beside the sound card drivers first, as it is by far the most predictable. If you recently upgraded your OS, try finding drivers that meeting the OS version that you hold. (for example XP drivers don't work on Vista and vice versa). If you upgraded your driver package, revert to an ahead of time state or try to download a driver that matches your hardware and OS.
